At Housecall Providers, we believe everyone deserves access to compassionate, high-quality medical care, even when leaving home is difficult. For more than 30 years, we have been a leader in nonprofit, home-based medicine, bringing primary care, palliative care, and hospice directly to where our patients live.

Today, Housecall Providers is part of Chapters Health System, a national nonprofit organization focused on caring for people with chronic and serious illness. Being part of Chapters gives our patients the benefit of a broader network of care, along with added knowledge, resources, and support as their health needs change.

Our care is personal and built around each patient’s needs, goals, and daily life. Our teams work closely with patients and families to manage chronic conditions, improve quality of life, and provide comfort and support through serious illness and the end of life.
