Accreditation from CHAP demonstrates that Housecall Providers Advanced Illness Care meets the industry's highest nationally recognized standards. The rigorous evaluation by CHAP focuses on structure and function, quality of services and products, human and financial resources and long-term viability.

Housecall Providers Advanced Illness Care provides community-based and telehealth palliative care support to individuals living with a serious illness in the Portland tri-county area as well as Clatsop, Tillamook, Columbia, Douglas and Jackson counties.
The advanced illness care team includes nurses, social workers, community health workers, a pharmacist, and a spiritual counselor who all have the flexibility to make home or telehealth visits. The team works closely with patients and their other health care providers, focusing on patients’ goals of care, symptom management, and care coordination. They identify and align additional community resources while providing support as the illness progresses.
