At your doorstep

A few months into our new home with Chapters Health System, the nation’s largest nonprofit provider of hospice and serious illness care, I am taking a moment and a deep breath amid the integration work to recognize everything it took to get us here, and everything still ahead as we bring our two organizations fully together.

Nearly two years ago, I began exploring whether a larger provider organization could help us achieve our sustainability goals while staying true to our mission and values. A year ago, Chapters Health clearly emerged as the front-runner. After moving through the definitive agreement phase last winter and the five months of integration work that followed, we officially became part of the Chapters Health family on July 31. I can honestly say that I haven’t questioned that decision. Every step, especially after meeting and working with their leadership team, confirmed it was the right one.

Yet none of this would have been possible without CareOregon. For the last nine years, CareOregon has worked side by side with us to expand access to compassionate, high-quality care for people across Oregon. That partnership positioned us to take this
step with Chapters Health, and CareOregon remains very much part of our ecosystem, not only because we continue to care for their members, but because our missions have aligned for decades, through shared innovation and ideas.

Housecall Providers is part of the Chapters Health’s West Coast network, which also includes affiliations with Hospice East Bay in Pleasant Hill, California; Hospice of Santa Cruz County in Santa Cruz, California; Nathan Adelson Hospice in Las Vegas; and Willamette Vital Health in Salem, Oregon. Joining these hospice and serious illness providers will let our work flourish as we share ideas, projects and processes, and benefit from the economy of scale this affiliation provides.

Chapters Health is excited to learn from our home-based primary care program and benefit from our 30-plus years of experience in this field.

This partnership also gives us access to additional resources for workforce recruitment, innovation, technology and fundraising, tools that will help us grow and meet the needs of our patients. Above all, this affiliation is about protecting the mission of Housecall Providers for years and decades to come.

Today, we are one step closer to a future where every homebound adult and every person living with serious illness has access to care that truly improves their quality of life. That has been and will remain our vision. Now, standing alongside partners who share it, we have the resources and the reach to make it a reality.

Thank you for believing in this work, and for standing with us as we carry it forward.

In gratitude,

Rebecca
