AIC Patient Information

Palliative care (pronounced pal-lee-uh-tiv), is specialized medical care for people with serious illness. It focuses on providing patients with relief from the symptoms and stress of a serious illness with the goal to improve the quality of life for both the patient and the family.

Patients can receive this specialized care with the Housecall Providers AIC Team. This is a team of specially-trained nurses, social workers, pharmacists and spiritual counselors who work together to provide an extra layer of support to patients

The Housecall Providers AIC Team will work closely with your primary care and specialty providers to provide symptom management, care coordination, medication assistance, and assist with access to community resources that may be available to you. This team will also provide emotional and spiritual support to you and your family and/or caregivers. The team will provide information to you about your illness so you can make decisions about your care and focus on what is most important to you.

Currently we serve patients with CareOregon Medicaid, CareOregon Advantage and Legacy/PacificSource Medicaid health insurance plans.
