Independence At Home (IAH) Demonstration Project

In late April 2012, HCP learned that we had been selected to participate in the three year national demonstration project Independence at Home (IAH). (The project has since been extended and will run though May of 2017.) This landmark legislation was part of the Affordable Care Act and sponsored in the Senate by Oregon’s own Ron Wyden. HCP was selected along with 15 other sites to treat 10,000 Medicare beneficiaries (project participants) who represent a sampling of the five percent of the patient population that utilizes nearly 50% of the Medicare budget. HCP termed our involvement in the IAH Demonstration as Leading the Way to highlight that we are the only site west of Texas to be selected, and to underscore that we have always implemented the style of care that the Center for Medicare & Medicaid Services (CMS) is now recognizing to be not only better for the patient, but also, when fully realized, has the potential to save Medicare billions.
