We believe that everybody should have the right medical care, delivered at the right place, at the right time. For many people who have difficulty getting to their medical appointments, Housecall Providers primary care is the right choice.
Holistic primary care services at home
Housecall Providers offers care to our primary care patients using a team-based model. This means that in addition to your primary care provider (PCP) you might also get care and assistance from a nurse, social worker, spiritual counselor, care coordinator and scheduler who make up your primary care team.
Our intention is to provide you or your loved one with just the right care to help meet your goals. Different members of the team may help you at different times as your medical condition changes. There may be times when you have one or two members of the team involved and other times when you have many more.
Our clinicians perform minor procedures in the home but also have the ability to connect patients with the services they may need such as in home blood draws, X-rays, ultrasounds, EKGs (electrocardiograms) or referrals to specialists. Our proactive coordinated care helps prevent emergency room visits and hospitalizations.
Here for our patients 24 hours a day
In-home primary care visits for established patients can be scheduled from Monday through Friday by calling our office at 971-202-5500. Your PCP is available by phone 9 a.m. to 5:00 p.m. Monday through Friday. After hours and weekend calls are handled by a rotation of our PCPs.
Patient portal
The Follow My Health Patient Portal is a secure and HIPAA compliant way for existing Housecall Providers patients to review medications, communicate with your primary care provider, or track progress through a treatment.
If you would like to sign up for the patient portal or are the health care proxy for a Housecall Providers patient, please speak to your provider or another of you primary care team members at your next visit to start the process. We would also like to invite you to review the portal’s terms of use agreement, as well as the answers to many frequently asked questions.
